Enrolling in MyHealth Online is easy. Using MyHealth Online you can view your medical record, renew prescriptions, request an appointment, view test results, pay your hospital and physician bills, and more.
Please fill out the form to request an Activation Code to be sent to you via postal mail.* This information is used to verify and match your records. All information is kept confidential.
Please Note: You must be at least 18 years old to request an Activation Code to MyHealth Online.
Setting up Proxy Access to Another MyHealthOnline Account If you are a parent looking for access to your child's medical record, or you are a caregiver looking for access to the medical record of the person you are caring for, you must have your own MyHealth Online account. In order to have a MyHealth Online account, you must also have a UVM Medical Center medical record number. To receive your own UVM Medical Center medical record number - and to set up your own MyHealth Online account - we recommend that you take a signed proxy authorization form to the clinic where the patient is being treated and the clinic will request a medical record number for you, as well as help you to set up a MyHealth Online account and proxy access. If you cannot go to a clinic, please mail the form to the IS Service Center using the address below:

*Important: For health information security purposes, all requests for a University of Vermont Medical Center MyHealth Online account must be verified. You will be mailed a letter with your Invitation Code to the address The University of Vermont Medical Center has on file.
